WELCOME

We are excited that you have chosen to join us by becoming a member of the Dr. Lee S. Cohen and Associates community.  We love making new friends.  All of us at Dr. Lee Cohen & Associates are looking forward to making your first visit easy and comfortable.  To that end, there is some information we'll need from you that can be taken care of before you come to our office. If you have a printer, please print out the packet below, fill it out, and bring it in with you on your first appointment.

INSURANCE

Please bring your insurance card. We will need a copy of it for our records. If your insurance plan requires that you have a referral from your primary care doctor, please make sure you obtain one before your appointment. We accept most of the major medical plans in our community. Please feel free to contact our office to verify your insurance plan.

All sports medicine patients should bring the following with you to your appointment as this will be extremely helpful:

• Shorts

• Tee shirt or tank top

• All sports footwear including spikes or cleats and insoles or devices worn for activity

• All relevant X-rays, MRI, or other diagnostic studies

As a new patient, you will receive our newsletters and updates, appointment reminders, and we hope that you may also choose to follow us on social media. As a welcome to our family, during your first visit, you’ll also receive one of our signature t-shirts and a drawstring gym bag.
